Patient Portal

A patient portal is a secured method to view and exchange select information. This includes:

  • View upcoming appointments
  • Accessing symptom monitoring tools to complete prior to follow-up visits
  • Ability to view and edit demographic information (ie insurance information)
  • Secured messaging that allows communication of non-urgent information

The address to sign up for and log on to the portal is:

Signing-up:  There are 2 ways to sign-up if you would like to utilize the portal after we begin working together –  Express Validation or Email.

  1. Express Validation – On your next visit we will provide you with a Validation Code and directions to set up your portal account.  The Validation Code will expire in 24 hours.

  2. Email – At your next visit provide me with your email and I will email directions to set up your portal account.  The email directions will expire in 72 hours.
